  • I agree that before starting this registration, I have read through theDonor Information and FAQs provided by AED's website to understand the requirements and how the egg donation procedure works.
  • I understand that I will be required to attend approximately 10 appointments on-time throughout a donation cycle, most clinic visits will be in early morning.
  • I understand that I will be required to take self-administered hormone injections for 2-3 weeks.
  • I am aware of the medical and psychological risks involved to donate my eggs.
  • I understand that my decision to become a donor is a serious matter and the Intended Parents place a tremendous amount of trust in me to follow instructions carefully and stay committed and responsible and do everything possible not to add any risk of failure for their attempted pregnancy.

Note: All donors will be required to answer this FDA Questionnaire at screening appointment. If you answer yes to any of the following questions, you may be disqualified to be an egg donor.

In the past 12 months have you
Had a blood transfusion?
Had a transplant such as organ, tissue, or bone marrow?
Had a graft such as bone or skin?
Come into contact with someone else blood?
Had an accidental needle-stick?
Had sexual contact with anyone who has HIV/AIDS or has had a positive test for the HIV/AIDS virus?
Had sexual contact with a prostitute or anyone else who takes money or drugs or other payment for sex?
Had sexual contact with anyone who has ever used needles to take drugs or steroids, or anything not prescribed by their doctor?
Had sexual contact with anyone who has hemophilia or has used clotting factor concentrates?
Female donors: Had sexual contact with a male who has ever had sexual contact with another male?
Had sexual contact with a person who has hepatitis?
Lived with a person who has hepatitis?
Had a tattoo within last 12 months?
Had ear or body/ear piercing within last 12 months?
Had or been treated for syphilis or gonorrhea within last 12 months?
Been in juvenile detention, lockup, jail, or prison for more than 72 hours?
From 1980 through 1996
Did you spend time that adds up to three (3) months or more in the United Kingdom ? (Review list of countries in the UK )
From 1980 to the present, did you
Spend time that adds up to five (5) years or more in Europe ? (Review list of countries in Europe .)
Receive a blood transfusion in the United Kingdom ? (Review list of countries in the UK .)
From 1977 to the present, have you
Received money, drugs, or other payment for sex?
Have you EVER
Had a positive test for the HIV/AIDS virus?
Used needles to take drugs, steroids, or anything not prescribed by your doctor?
Used clotting factor concentrates?
Had hepatitis?
Had any type of cancer, including leukemia?
Had any problems with your heart or lungs?
Had a bleeding condition or a blood disease?
Had sexual contact with anyone who was born in or lived in Africa ?
